Is this HIPAA-compliant?

The poll itself doesn't collect protected health information — it's a rating and an optional open-text field. As long as you don't ask about diagnosis or treatment details in the question, you stay outside HIPAA's scope. For stricter compliance needs (including BAA), speak to us before rolling out.

Will patients actually fill in a survey after their visit?
